Using Your Report With Your Doctor

How to use your report to have a better doctor visit

Now you don't have to remember every symptom you have, all the medications you take, or what to discuss with your doctor.  Your report includes this information.  Bring both pages with you to your appointment.  Page one has important information to discuss with your doctor.  Page two has the medical information your doctor needs... make sure your doctor gets page two.

How your doctor uses your report

Page two is what you give to your doctor.  It contains your health history, current symptoms, medications and their dosages - all in standard medical terms that your doctor is used to seeing.  This is the information that helps your doctor help you.  Your doctor can quickly review your report and then focus on telling you how to get better or what to do next.  Your doctor will appreciate you bringing your report to give him or her as much information as possible about your medical condition.
